Your medical record is a confidential document and your privacy will be respected at all times. For patients seen in the Sydney CBD clinic or via Online Consultations, the medical information recorded through the questionnaires, consultations and any other data that may be scanned into your medical record – is securely stored within the “Living Matrix” specialised Functional Medicine software (a collaboration with the Institute for Functional Medicine) that Dr Gajjar utilises. This is a secure site and no other person other than Dr Gajjar will be able to access this information. Dr Gajjar will not disclose any information with other health professionals or third parties (e.g insurance companies) unless you have given your formal written consent.
For members of the public who voluntarily provide their email address (e.g through signing up for e-books, attendance at events – workshops or retreats) this information will not be shared with any third parties.
