*NOTE : Please be advised of reduced hours this year – Wednesday is the only day in clinic and Telehealth on the other days.

New patient appointments are limited, with priority for existing patients. If capacity is reached, new patient registration may temporarily close. If no new patient appointments are available on the calendar, please email and we will add you to the waiting list. We apologise for any inconvenience *

Appointments

How to book an appointment:

Please use the “book appointment” option on this website.

Available dates will be shown. Continue scrolling along to future dates until you see availability (blue circle). Click on the appointment type, day and time required.

For future appointment bookings, you will have access to your online patient portal; we also advise you download the mobile appplease use this in future for all bookings. 

Note: 

Face to face appointments – Wednesdays ONLY

Telehealth appointments –  Monday to Thursday available

For clinic appointments – Please feel free to bring in a notepad to take notes. Recordings are not permitted.

How to pay for an appointment

Credit card details are required for all bookings. You can pay at time of booking or the fee will be charged at the end of the consultation. If bank transfer is preferred, let us know during or just after the consultation (details on invoice).

Medicare rebate

Once you have attended in person, for the next 12 months, medicare rebates apply for telehealth appointments. Hence, where possible, try to come in person for your first appointment. Once you have attended in person, either as new or follow up patient, medicare rebates will apply for future telehealth appointments.

Once payment is received, after the consultation, Medicare will be notified and your rebate will be credited into your nominated bank account (eligibility criteria apply).

If you are over the medicare threshold, a higher amount of the gap will be refundable. Allow up to 3 working days.

New Patients

All new appointments are one hour in duration and available in person (preferred)  or as telehealth. Please note – no medicare rebate for initial telehealth appointment.

Initial consultation – 60 m

Comprehensive review  including the following:  (total time approx 100 mins).

  1. Review of completed questionnaires prior to appointment
  2. Consultation (60m)   – thorough history and relevant examination; we will address your specific queries and discuss relevant steps forward.
  3. Management plan  – sent to you via email, within 3 days of the consultation, outlining (as appropriate) –
  • Lifestyle advice
  • Referrals for investigations
  • Supplement/ medications prescriptions
  • Links/Resources for further information

After the consultation is confirmed, you will be sent the following forms:

New patient questionnaire

Medical symptoms questionnaire (MSQ)

Clinic terms and conditions

Consent for Integrative/Functional medicine

Please complete the forms at least 3 days before the appointment- failure to do so may result in rescheduling/cancellation.  

Follow Up Consultations

Follow up appointments are usually for 30 minutes. In some cases, if shorter or longer appointments are appropriate, this will be advised. Once you have attended in person, for the next 12 months, medicare rebates apply for telehealth appointments.

Telehealth appointments –  The video link is within the patient portal. Go to your appointment details and click on  “join session“.

If there are internet connection issues, Dr Gajjar will call you on the phone.

    Email policy

    Most patients will not have the need to email. It is not designed for routine queries or discussions. Please bring all your non-urgent questions, requests, updates of your history or results to your actual appointment.

    If you do need to email between appointments, email to admin@dramygajjar.com – and not to Dr Gajjar’s social media or other websites.

    Simple emails requiring yes/no and brief responses will be answered within 3-5 working days.

    Lengthy enquiries/ multiple questions –   book a short phone consult.  For email correspondence,  use the “Request Form” link on the website.  (min charge $20).

    Please note – emails are not answered on weekends and public holidays.

    General admin

    Out of consultation requests – please use the  “Request Form” option on the website for repeat prescriptions, medical certificates, referral letters, pathology requests,  other forms and email correspondence.

    Results – please do not call for your results – arrange for an appointment to discuss these. If an appointment is not available in the near future, it may be possible to book in a short phone consult.  If anything is urgent, we will contact you.

    Waiting  list: if you are unable to book your preferred time,  email at admin@dramygajjar.com and advise us so that we can add you to a waiting list and inform you accordingly. Please also check online to see if  any available times are available (in case of extra clinics added, cancellations etc). 

    Cancellation policy

    The cancellation policy requires a minimum of 24 business hours’ notice (Monday to Friday: 9am to 6pm). We have patients who are waiting for an appointment that would be happy to fill your appointment. The cancellation fee is the cost of your booked appointment time.
